I thought I had completely signed up, and clicked on the app. I was sent to an email, already filled in with the subject (Amgen Migraine Voice Journal Consent Document) with the consent.pdf attached, which looks like they would like the .pdf sent to them. Great. Not filled in? The address. Did anyone else get this? With an address, maybe?"
