Getting a “Checking In” form from SSA

As I’m in bed recovering from surgery I received a letter from SSA “checking on me “. Now I remember when @hollyharding lost her long term coverage all together, but this was different. my disability was approved 1/1/2020 and 4 years later I get a letter just asking me if I’m doing any work, seeing any doctors, having any procedures done. The really weird thing was it was on a scan tron form so I couldn’t even say what all I wanted to say. Anyone familiar with these? And if so, what’s next? Thanks!!!

  1. Hi - Good to hear from you. I have not encountered the use of a scantron form from SSDI. My disability loss was not from SSDI but through a private disability insurer, by the way. What a pain that there was no space to properly fill out what you needed to say. Do you have a contact at SSDI? I have a name and number of a woman there who was my case manager but it is from years ago and I honestly doubt she still works there. If you have a contact over there, I might recommend you call or write a letter offering the information you wanted to express (that you continue to do everything you can do find relief and improve your condition but to no avail - showing all of the examples of your proactivity) explaining there wasn't room to do so on their provided form. Of course, take care to share only what's relevant and make a copy of everything you submit to them. Otherwise, I suppose you wait to see if they reach out for more information. Super strange.     1. I remember yours was through your employer provided benefits. I’ve never received any follow up forms since being declared disabled which I had mentioned here on the site before and someone told me it was just a matter of time. The form asked if I had worked since 2022, if I had seen a doctor since 2022 or had a surgeries or procedures. I found a link to do it online but again it gave very limited space. I’ve tried to call twice but the wait times are over an hour. I will try to call tomorrow right at 8am. I just want to see why I received the letter and such. If you do have a contact, I’d be thrilled to have the information. Maybe I’ll get lucky and they are still employed! I appreciate the support I always receive from you.

    2. I'm really curious to know more about this form. Just to confirm, when you say it was a scantron sheet, do you mean it was like a multiple choice, fill-in-the-bubbles situation? That would be interesting, but more importantly, it seems ineffective to lock people into specific answers like that.       1. So it’s not like a college exam scantron! But it’s squares and they want full answers but you don’t have enough room. I found it on the ssa.gov site and it was the same. Even the “Remarks” box stopped you after so many letters… think back to grade school and the standardized testing and having a really long last name that did fit! I’m going to try to email it to you so you can see what I mean

      2. That's sooooo weird. I think I get what you're talking about, though. Almost makes me wonder if the forms are "read" by computer, recorded in your file and never seen by an actual human. 😐 Now I really want to know what you can find out!
